A knockdown rebuild is treated the same as a new build for this purpose, so collars and any under-slab work still need to go in before the pour, even where the previous house had no protection at all.
Yes, a new extension slab or footing still needs a compliant termite management system where it forms part of the new work. We also check the join between the new footing and the existing house, since that connection point is a common place for a gap to be left if it is not planned for in advance.
